We offer a variety of programs at several locations. 

You MUST have an appointment for your assessment.
If you are a self-referral, follow the instructions of our office.

If you are referred by your employer, we need your authorization form.

If you have been referred by the Safe Children Coalition, Department of Corrections or DUI School, we MUST have your full paperwork prior to your assessment.

The assessment determines if you require treatment;
and if so, which program is most appropriate for you.
